Practice Area

Patients are encouraged to register at a GP Practice within their local area, however the Grange allows patients from all areas in England to register with the practice. Patients outside of the practice area must be aware that they will not be eligible for home visits and service provision may vary for those outside of the practice boundary.

Copy of these forms are also available at the practice reception.
  • Download and Print the Registration Form and complete with only using BLACK INK.
  • Provide as much information as possible to help us obtain your medical records quickly.
  • A form of Photo ID is helpful but not necessary.
  • You will be asked to sign the form to confirm that all details are correct.
