New Patient Registration
The Practice is NOW ACCEPTING NEW PATIENTS.

To register at a Practice, please print off and complete the following for each person who wishes to register:
Please complete the above in BLACK INK and hand in at reception. Copies of these forms are also available at reception.
Please provide as much information as you can when completing the above forms. This will help us to obtain your medical records as quickly as possible.
You will be asked to sign these forms to confirm that all the details are correct.
Before arranging a registration appointment, please ensure you can provide the following documents:
- Completed registration forms (as above)
- Confirmation of address (Utility bill, Bank Statement etc)
- Photographic Identification

Practice Catchment Area
The practice area covers a large geographic area, which includes:

Removal of patients who live outside the practice catchment area:
Following the departure of a full-time GP 18 months ago, the practice has been unable to recruit a replacement GP despite our best efforts. We are therefore working with one GP short.
If a registered patient lives outside our practice catchment area then we would be contractually obliged to visit the patient as necessary. This can take a considerable time out of a GPs day, which is time that can be used to deal with several patients who live within the catchment area. Our motivation is to continue to provide a high quality service to our patients.
We are implementing new models of care but feel we have no option but to remove patients from our list who live outside the practice catchment area. We have had to take this tough decision, as daily workloads of existing clinicians are now extremely high while we seek to deliver access for both urgent and routine care from the present workforce.
We have followed due process in this matter and have consulted with our Patient Group, NHS England and Greater Huddersfield CCG, all who have given their support.
The practice is therefore asking patients who live outside the catchment area to find an alternative GP closer to their home. We are writing to these patients and given them guidance about registering with another practice. This is being done in a non-discriminatory manner, with palliative patients being the only exception.
We regret that this decision has affected patients who have been with us for a long period, as we value the doctor patient relationship and continuity of care, but this will enable us to provide the level of care to patients in our practice area, which we believe essential.
